When filling out a records request form, please keep the following in mind: All students will need to fill out a THS Student Records Request Form. If THS is mailing the transcript, the correct address to the college, university or scholarship will need to be provided. Allow four business days for processing.
If you are picking up a form at THS: To pick up your form, please see the THS front office. If you are at THS and need assistance or have questions on transcripts, please call 215-7012 and the office staff will assist you.
If you are obtaining a form online: The records request form is provided below. Once completed, please return the completed form to Temple High School
